It comes with you. Fine, if it is not fixed to the building and if it makes sense at the new address. Many signs are one or the other but not both.
It stays. Also fine, and often the sensible outcome for anything screwed to brickwork.
It gets replaced. Which means ordering something, and sign-makers work to their own timescales rather than yours.
The third is the one that needs the most notice and gets the least. If your new address needs a sign, start that conversation early, well before the move rather than during it.

Anything with your address on it needs updating: the website, the listings people find you through, the map pin, the signage inside the building, and whatever a visitor would check the night before coming to see you.
The map entry is worth particular attention. It is the thing most visitors actually use and the thing most often still pointing at an empty office three months later.
